This form has been a source of frustration for years. Although only used under special circumstances. there is no question that it has discouraged many persons overseas from seeking to visit the United States. All visa applicants must still fill out the socalled short form and. where asked to do so. must also take an oath that the information contained therein is true. In the short form. they are required to indicate whether they are ineligible under our laws. which exclude persons for a number of specific and precise national security. health. and related reasons. Should the consul desire to ask further questions pertinent to these provisions. he may. of course. do so in connection with the applicants answers on the short form.
